Harvey James, born  the ninth day of December, eighteen hundred and ninety three, Pheba Ethel May, born  the twenty first day of January, eighteen hundred and ninety seven, Lilian Winnie, born March the first, in the year one thousand nine hundred, Abigail Margaret, born the twenty first day of February, one thousand nine hundred and two, children of Oscar David Johnson, of Dunham Township, farmer, and Ella May [Gibson] his wife, were baptized at the Stone School House near Dunham Lake, on the fifth Day of August, one thousand nine hundred and two, in the presence of the Parents and then of me

Dans le cas d'un acte multiple, l'information relative aux personnes concernées est minimale et incomplète comme dans le présent exemple. Noter l'absence de mention de parrains et de marraines et le fait que ces baptêmes ont eu lieu dans une école et non à l'église comme chez les catholiques.
